Cassandra isn’t Charles’ twin. She’s Xavier’s right brain.
Charles has always been a super-rational, disciplined, Apollonian genius rather than a creative, intuitive one. I believe this is the result of an early struggle for dominance between the two lobes of his ultra-powerful mutant brain. The left brain won, and “killed” Xavier’s emotional self by burying it deep in his subconscious, where it smothered and festered, growing malign and hateful.
All that I’m pretty sure about. What I think happened next is that the suppressed side of Xavier bided its time until it discovered an incredibly powerful mutant, one of the first of the “next generation”. Perhaps the mutant had just been born, or had just entered adolescence, or had just been injected with the gene boosting serum. “Cassandra” then abandoned Xavier’s body entirely and took possession of the new mutant as the first step in a elaborately planned scheme to take Xavier’s body.
